[Desk Settings](https://knowledgebase.clearooms.com/settings?hsLang=en-gb#desk-settings) # Custom Bookings Forms ## Collect Key Information with Custom Booking Forms - #### Custom Booking forms allows you to set questions for users to complete when booking a desk. Make them optional or mandatory and block bookings based on answers provided. These are Ideal for health questionnaires to keep the office safe.  #### **Creating custom questions** Open [Settings \> Hot Desks \> Custom Booking Fields](https://portal.clearooms.com/settings/hot-desks/custom-booking-fields) and click "Create New Custom Booking Form"  - **Name: **This is for your reference only, enter a memorable name for the question - **Type: **This is a dropdown or Check Box, Text Box or Yes/No Field.  - **Field Label: **This is the question you are asking the user when they book a Desk - **Order: **Leave blank for the system to order or enter values to force an order - **Required Field: **Check box that makes the question mandatory or not - **Restrict booking depending on value?**: This is optional, if you select it you will see the option:  - **Answer must be the following to make the booking: **Select what the correct answer is to allow bookings to be processed  - **Apply To Specific Offices: **Open and select specific Offices or leave off for all Offices - **Apply to Specific Desks: **Open and select specific Desks or leave off for all Desks You can repeat the above steps to create multiple questions, each question can be specific to certain Desks or Offices if you like.  #### **General Settings** Once you have some questions these two options become relevant - **Send email if user enters invalid information to custom fields?: **If you turned on "*Restrict booking depending on value?*" when users fail to enter the correct answer an email will be sent to the nominated person set here.  - ** \*  Use custom message if user fails to enter valid values?: **Also relevant to the restriction of bookings, this is the message that users are shown when entering incorrect values, you can edit what the users see here to make it more relevant to the questions you have asked.  \*Example: If your question is ''please confirm you have read the policy'' the message to those that do not check it could be ''You cannot book Desks in the Office without confirming the Office Policy.
